Anesthesiology

Includes physician, doctor, or provider's time to complete a visit or procedure that includes evaluation and management of your care.

Doctor

Your doctor may determine the need for medical clearance which may include diagnostic testing prior to surgery. In this case, you may receive a separate charges for pre-op clearance.

Does Not Include Pre-Operative Clearance

Includes supplies, equipment, devices needed during treatment, and service being rendered. 

Facility, Equipment & Supplies

Post-operative visit(s) related to the patient's recovery from the procedure.

Post-Operative Visit(s)

Service with provider either day before or day of surgery in which patient is evaluated, informed, and prepared for surgery.

Pre-Operative Visit

Physician order required.Please do not order until "Orthopedic Surgeon / Doctor Visit" with chosen doctor has been completed.

First Post-Operative Visit

Devices used to temporarily align, support, stabilize, and protect parts of the body as they heal after surgery.

Orthopedic Braces or Supports

Includes orthopedic support (boot) and post-operative x-rays.

X-Ray included, does not include advanced imaging.
