Uncontrolled Diabetes Program in Kansas

Uncontrolled Diabetes Program is for patients with diabetes and an A1c over 8%. At the end of 12 months, he/she will obtain appropriate knowledge of disease state and lifestyle efforts needed to achieve and stay in control, have an A1c under 7% (if clinically appropriate), medications will be optimized, with future access to ongoing care via Diabetes Control Maintenance Program if metabolic target is achieved.
