The "Prevent Diabetes Program" is for patients with first-degree relatives with diabetes or for people with pre-diabetes. Patients will gain appropriate knowledge about modifiable factors to prevent onset of diabetes and access to ongoing support to implement changes to postpone onset of diabetes.
