The "New Onset Diabetes Program" is for patient diagnosed with diabetes within 6 months prior to referral. During the 12 months, he/she will obtain appropriate knowledge of disease state & lifestyle efforts needed to achieve & stay in control; medications will be optimized with future access to ongoing care via "Diabetes Control Maintenance Program" if metabolic targets achieved.
